Purpose of the Role:

We are seeking a dedicated Social Worker to engage with adults and carers, promoting well-being, independence, and access to community resources. Your role will involve intensive support during crises, empowering individuals to regain control and plan for the future.

Through meaningful conversations and assessments, you will develop support plans, enabling adults and their carers to lead fulfilling lives within their communities.

Main Activities & Responsibilities:

  • Utilise a strength-based approach to support adults with diverse care needs, minimising identified risks.
  • Collaborate with professionals to facilitate multi-agency approaches, promoting independent living for adults and carers.
  • Ensure compliance with statutory requirements, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
  • Engage in reflective practice and peer support, actively contributing to team dynamics.
  • Identify and address safeguarding concerns, adhering to the Making Safeguarding Personal approach.
  • Represent the authority in court proceedings and other official capacities as required.

Additional Duties Include:

  • Assess eligibility under the Care Act 2014 and facilitate access to available resources.
  • Develop creative support plans focused on achieving outcomes and maximising independence.
  • Analyse information from various sources to inform decision-making.
  • Collaborate with the Adult Safeguarding Team to promote welfare and address concerns.

Requirements:

Experience:

  • Demonstrated social work experience, particularly with older adults and individuals with disabilities.
  • Proven ability to engage users and carers in their care arrangements.
  • Experience working collaboratively with health and social care agencies.

Knowledge, Skills & Abilities:

  • Thorough understanding of social work theories, legislation, and strength-based practice.
  • Excellent communication skills, both verbal and written.
  • Strong organisational and multitasking abilities.
  • Proficiency in IT and support planning techniques.
  • Capability to work effectively within multidisciplinary teams and build partnerships.

Qualifications & Development:

  • Professional qualification in social work and registration with Social Work England.
  • Desirable: Relevant post-qualifying awards and proficiency in Microsoft Office or Level 1 Essential Digital Skills.
