The Role:

We’re excited to offer two positions for experienced Social Workers specialising in Mental Health and Learning Disabilities within Adult Social Care. Currently, Adult Social Care Commissioners are actively involved in developing a Social Care Mental Health Recovery Pathway and a Supported Living Project. As part of this initiative, we require experienced professionals to collaborate closely with coordinated Social Work Reviews to assess current service provisions and facilitate moves towards more independent living where suitable. To support these endeavours, we’ve established a dedicated Pathway Planning Team.

Key responsibilities for the successful candidates, in partnership with our commissioners, will include:

  • Providing expert Social Work input to the pathway planning team, demonstrating proficiency in needs assessment, reviewing support plans, and a strong grasp of The Care Act, Continuing Health Care, and s.117 Aftercare provision (MHA).
  • Assisting in the development of recovery pathways for Mental Health to enable greater independence for individuals.
  • Conducting comprehensive reviews of packages for individuals, assessing potential for relocation or change of services.
  • Identifying opportunities for individuals to transition to alternative community living arrangements and supporting such transitions.
  • Contributing to the re-modelling, de-registration, or decommissioning of services where necessary.


  • Degree or equivalent within Social Work
  • Current Social Work England registration
  • Previous UK Social Work experience working within Adult Services
