Purpose of the Job:

As an Adults’ Social Worker, your role is pivotal in promoting the well-being of adults and carers while enhancing their independence and community engagement. You will provide intensive support during crises and facilitate future planning, fostering meaningful connections and support networks within the community.

Main Activities & Responsibilities:

  • Engage with adults utilising a strength-based approach to address various care and support needs.
  • Foster multi-agency partnerships to enable independent living for adults and/or their carers.
  • Ensure compliance with statutory regulations, including the Care Act 2014 and related legislation.
  • Engage in reflective practice, contributing actively to team dynamics.
  • Identify and address safeguarding concerns, adhering to established protocols.
  • Represent the Council in legal proceedings when necessary.
  • Advocate for service users’ perspectives in decision-making forums.
  • Conduct assessments and support plans aligned with the Care Act 2014.
  • Cultivate collaborative relationships with external agencies and the local community.

Additional Duties & Requirements:

  • Develop creative support plans to enhance adults’ independence and well-being.
  • Conduct comprehensive analysis to inform decision-making processes.
  • Ensure adherence to legal frameworks regarding deprivation of liberty.
  • Collaborate with the Adult Safeguarding Team to address safeguarding concerns.
  • Facilitate discussions on Continuing NHS Healthcare eligibility.
  • Maintain accurate records and participate in reflective practice and supervision.
  • Exercise financial decision-making within established guidelines.
  • Fulfil continuing professional development obligations.

Additional Information:

  • £3k MFS subject to review
  • Flexibility to travel across the county, including areas with limited public transport.

Requirements:

Experience:

  • Proven track record in social work, particularly in assessment and support planning.
  • Experience working with diverse adult populations, including older adults and individuals with disabilities.
  • Demonstrated collaboration with health and social care agencies.
  • Utilisation of community resources to create sustainable care plans.
  • Evidence of Continuing Professional Development.

Knowledge, Skills, and Abilities:

  • Familiarity with Social Work theories and legislation.
  • Clear and effective communication skills.
  • Strong interpersonal skills and ability to manage multiple priorities.
  • Proficiency in IT.
  • Competence in assessment and support planning.
  • Ability to collaborate within interdisciplinary teams and external agencies.

Qualifications/Training & Development:

  • Professional qualification in social work.
  • Current registration with Social Work England.
  • Desirable: Relevant post-qualifying award and IT proficiency.
